This report summarises the support ALONE has provided as part of the HSE Enhanced Community Care scheme in Q3 2023. The main purpose of the report is to demonstrate how ALONE has supported the HSE to achieve its vision of increased person-centred community health supports in Q3 2023.
The report demonstrates how ALONE is central to ensuring the success of Sláintecare, by creating a model of integrated care for older people, ensuring older people can access a range of services that enhance their physical and mental health.
The six key objectives of the ALONE model within the ECC programme are outlined in the report, namely, building local support networks, supporting independent living, emphasising a preventative care approach, driving national data collection and research, and delivering person-centred care integration and collective service delivery.
Key findings from the report include:
Loneliness remains a significant issue, with 63% of older people assessed by ALONE reporting feelings of loneliness, up from 60% in Q2 2023.
63% of newly supported individuals were living alone, while 50% had physical health concerns, an increase from 48% in Q2 2023.
Mobility issues were reported by 34% of individuals, compared to 31% in Q2 2023.
Among homeowners, 32% faced home-related difficulties, slightly less than the 37% of non-homeowners with similar issues.
In Q3 2023, ALONE conducted 12,564 interventions, marking a 53% increase from Q3 2022. On average, each person received 3.5 interventions, with the range spanning from 1 to 36. Of the 2,230 individuals assessed during the quarter, 96% received some form of intervention from ALONE.
